Clinical pharmacists work directly with physicians, other health professionals, and patients to ensure that the medications prescribed for patients help them achieve the best possible health outcomes. Pharmacists establish relationships with patients to ensure the appropriateness of medication therapy and patients’ understanding of their therapy, and to monitor the effects of that therapy.
